Dear Frost Middle School families,
We are pleased to share that Lawrence Public Schools, in conjunction with the City of Lawrence Public Health Department’s Mobile Vaccine Clinic, is hosting a series of COVID-19 vaccination clinics in September and October. The clinics here at the Frost Educational Complex will take place on September 9 and September 30 from 1-7 pm.
This free, voluntary opportunity is open to all our students and their family members 12 years old and over. No appointment is necessary and parents or guardians do not need to accompany their child, but consent is required for anyone 17 and younger.
You may provide consent any of the following ways:
- Return the consent form accompanying this letter with your child, completed and signed. Additional paper copies can be obtained at our main office or can be printed by visiting www.LawrencePublicSchools.org.
- Provide consent online at www.LawrencePublicSchools.org. You will also find the full district clinic schedule and other information.
- Parents may accompany their child to the clinic after school hours, and complete the consent form on site.
Students must submit the completed form before or at the clinic, or have it submitted online in advance, to receive the vaccine.
You can learn more about COVID-19 vaccines -- including specific information regarding children and teens -- at www.CDC.gov.
LPS is deeply grateful to the City of Lawrence Public Health Department for their support and collaboration, and we hope you find this opportunity helpful. If you have any questions, please do not hesitate to contact us.
Thank you,
John Patterson
Principal
